The organization of admission of patients in outpatient care and medical care at home is regulated by the Ministry of Health of the Republic of Tatarstan and the internal rules of the medical organization. In order to streamline the provision of routine medical care, patients are recorded, including in electronic form.
The order of sending patients to consultative polyclinics is established by the Ministry of Health of the Republic of Tatarstan. When sending a patient an extract from the medical card of an outpatient is issued in accordance with regulatory documents.
When providing medical care, it is provided that:
The organization of admission of patients in out-patient settings (pre-recording, self-recording of patients for out-patient reception) and the procedure for calling a doctor at home (the indication of telephones that register calls for a doctor at home, the convenient mode of operation of the registry) and the provision of medical care at home are regulated by the Ministry of Health Republic of Tatarstan and the internal rules of the medical organization. In order to streamline the provision of routine medical care, patients are recorded, including in electronic form.
When providing medical care, it is provided that:
the waiting period for the provision of primary health care in an emergency form should not exceed two hours from the moment the patient is referred to a medical organization;
the waiting period for the district doctor, the district pediatrician, the general practitioner (family doctor) should not exceed 24 hours from the time the patient contacts the medical organization;
the duration of consultations of medical specialists in the provision of primary specialized health care in the planning form should not exceed 14 calendar days from the date of the patient’s application to the medical organization;
the term for diagnostic instrumental (radiographic research, including mammography, functional diagnostics, ultrasound studies) and laboratory tests for primary health care should not exceed 14 calendar days from the date of their appointment;
the period of computed tomography (including single-photon emission computed tomography), magnetic resonance imaging and angiography in primary care should not exceed 30 calendar days from the date of their appointment.
Maintaining medical records
Medical records in medical organizations providing medical care on an outpatient basis are carried out according to the forms and order of their filling approved by the order of the Ministry of Health of the Russian Federation of December 15, 2014 No. 834n “On the approval of uniform forms of medical records used in medical organizations providing medical assistance in outpatient settings, and procedures for their completion. ”
In the medical chart of an outpatient, the dates of appointment and consultation and (or) study are indicated.
Consultations of medical specialists are carried out in the direction of the attending physician of the medical organization providing primary health care where the patient is attached.
For each patient in the medical organization or its structural unit providing medical care on an outpatient basis, no matter how many doctors are treated, one card is filled.
Medical cards of outpatients are stored in a medical organization. The medical organization is responsible for their safety in accordance with the law.
The patient or his legal representative has the right to get acquainted with the medical documentation reflecting the state of his health in the order approved by the order of the Ministry of Health of Russia of June 29, 2016 No. 425n “On approval of the procedure for acquaintance of the patient or his legal representative with medical documentation reflecting the patient’s state of health”.
The patient or his legal representative has the right, on the basis of a written application, to receive medical records reflecting the state of health, their copies and extracts from medical documents in the manner established by the Ministry of Health of the Russian Federation.
The issuance of medical certificates is carried out in accordance with the procedure approved by Order No. 441n of the Ministry of Health Care and Social Development of the Russian Federation of May 2, 2012 “On Approving the Procedure for the Issue of Medical Information and Medical Reports by Medical Organizations”, without charging the patient’s personal money (legal representative).
Conditions and procedure for rendering consultative and diagnostic assistance at the State Unitary Enterprise “RKB MZ RT”
The order of organization of the reception and the mode of operation of the consultative polyclinic are approved by the order of the chief doctor of the GAZR “RKB MZ RT” and are provided to visitors for information at information stands in the lobby of the polyclinic. Primary health care is organized and provided in accordance with the procedures for the provision of medical care (by profiles) and on the basis of medical care standards approved by the Ministry of Health of the Russian Federation, as well as in accordance with clinical guidelines and guidelines, and other normative legal documents. The scope, timing, location and timeliness of diagnostic and treatment activities shall be determined by the attending physician and indicated to them in the medical chart of the outpatient patient.
The provision of medical assistance by a specialist doctor in an outpatient clinic includes:
examination of the patient;
setting a preliminary diagnosis, drawing up a plan for examination and treatment, setting a clinical diagnosis, recommendations on issues of working capacity and the regime of life;
preparation of medical documentation;
providing the necessary information to the patient about the state of his health and explaining the procedure for conducting medical diagnostic and preventive measures;
emergency and urgent measures in the scope of the first medical aid and organization of hospitalization of the patient in case of immediate threat to life and the need for round-the-clock medical supervision.
The direction of the patient for planned hospitalization is carried out by a specialist doctor of the polyclinic of the “RKB MZ RT” in accordance with clinical indications, requiring hospital treatment, active therapy and 24-hour medical supervision. Prior to sending the patient to the planned inpatient treatment, a pre-hospital examination should be conducted in accordance with regulatory documents. In the direction of the established form, the objective survey data, the results of additional studies, should be included.
